FODAC Griffin helps connect people with refurbished medical and mobility equipment. Here’s how the local process works.
Call first and make an appointment.
Call 470-706-9626 to tell the Griffin team what equipment you need, check current availability, and make the required appointment.
Complete the request form before your visit.
Download the official FODAC Equipment Request Form below, print it, and fill out as much as you can before your appointment. This saves time when you arrive.
Bring your form and identification.
Bring the completed form and your driver’s license or state ID to your appointment. FODAC asks for identification each time you visit.
First-time recipients: bring $25 cash.
The one-time registration fee at Griffin is $25 CASH ONLY. The Griffin location does not have a card machine. Once registered, there is no additional charge for equipment received.
Meet with the Griffin team.
Work with the team to select appropriate equipment from current donated inventory. Availability changes; calling does not guarantee that a particular item will still be available at pickup.
