Before you visit
Visits are by appointment only. Call first to discuss equipment availability or an item you would like to donate.
For equipment assistance, download and complete the official FODAC request form before your appointment when possible. Bring the completed form and your driver’s license or state ID. First-time recipients must bring $25 CASH ONLY; Griffin does not have a card machine. Once registered, there is no additional charge for equipment received.
General nonmedical inquiry
